e35dfe864d avcodec/avcodec: Add FFHWAccel, hide internals of AVHWAccel
This commit is the AVHWAccel analogue of commit
20f9727018: It moves the private fields
of AVHWAccel to a new struct FFHWAccel extending AVHWAccel
in an internal header (namely hwaccel_internal.h).

80cc9b81de avcodec/nvdec_(mjpeg|vp8): Constify AVHWAccels
The discrepancy between the actual definition and the declarations
in hwaccels.h is actually UB.

6f7d3bde11 avcodec/vp8, vp9: Avoid using VP56mv and VP56Frame in VP8/9
Instead replace VP56mv by new and identical structures VP8mv and VP9mv.
Also replace VP56Frame by VP8FrameType in vp8.h and use that
in VP8 code. Also remove VP56_FRAME_GOLDEN2, as this has only
been used by VP8, and use VP8_FRAME_ALTREF as replacement for
its usage in VP8 as this is more in line with VP8 verbiage.

This allows to remove all inclusions of vp56.h from everything
that is not VP5/6. This also removes implicit inclusions
of hpeldsp.h, h264chroma.h, vp3dsp.h and vp56dsp.h from all VP8/9
files.

(This also fixes a build issue: If one compiles with -O0 and disables
everything except the VP8-VAAPI encoder, the file containing
ff_vpx_norm_shift is not compiled, yet this is used implicitly
by vp56_rac_gets_nn() which is defined in vp56.h; it is unused
by the VP8-VAAPI encoder and declared as av_unused, yet with -O0
unused noninline functions are not optimized away, leading to
linking failures. With this patch, said function is not included
in vaapi_encode_vp8.c any more.)

83c7ac2e47 avcodec/nvdec: Explicitly mark codecs that support 444 output formats
With the introduction of HEVC 444 support, we technically have two
codecs that can handle 444 - HEVC and MJPEG. In the case of MJPEG,
it can decode, but can only output one of the semi-planar formats.

That means we need additional logic to decide whether to use a
444 output format or not.

0e93694e64 avcodec/nvdec: More effort to make vp8 compile with gcc < 4.6
I'm told my prefix work-around wasn't enough to make it compile,
although I'm not sure why; I did some basic testing and that
approach appeared to work, but I'm not in a position to do a
full compile on CentOS 6 so I can't be sure of anything.

I have had it confirmed that the additional change to not use
named initialisers is enough to make it compile, so let's
throw that into the mix too.

b93d96a07b avcodec/nvdec: Make vp8 initialisation more 'compatible'
Ancient versions of gcc (pre 4.6) can't directly initialise
members of anonymous inner unions/structs by name.

Unfortunately, RHEL 6 shipped with one of these ancient
versions and so we're stuck with it until approximately the
heat death of the universe.

Putting explicit braces into the initialisation is possibly a
work-around but the behaviour there was never fully understood
before direct initialisation was made to work.

So, this may or may not work.
