4243da4ff4 avcodec/codec_internal: Use union for FFCodec decode/encode callbacks 
This is possible, because every given FFCodec has to implement
exactly one of these. Doing so decreases sizeof(FFCodec) and
therefore decreases the size of the binary.
Notice that in case of position-independent code the decrease
is in .data.rel.ro, so that this translates to decreased
memory consumption.

ce7dbd0481 avcodec/codec_internal: Make FFCodec.decode use AVFrame* 
This increases type-safety by avoiding conversions from/through void*.
It also avoids the boilerplate "AVFrame *frame = data;" line
for non-subtitle decoders.

20f9727018 avcodec/codec_internal: Add FFCodec, hide internal part of AVCodec 
Up until now, codec.h contains both public and private parts
of AVCodec. This exposes the internals of AVCodec to users
and leads them into the temptation of actually using them
and forces us to forward-declare structures and types that
users can't use at all.

This commit changes this by adding a new structure FFCodec to
codec_internal.h that extends AVCodec, i.e. contains the public
AVCodec as first member; the private fields of AVCodec are moved
to this structure, leaving codec.h clean.

a688f3c13c avcodec/internal: Move FF_CODEC_CAP_* to a new header codec_internal.h 
Also move FF_CODEC_TAGS_END as well as struct AVCodecDefault.
This reduces the amount of files that have to include internal.h
(which comes with quite a lot of indirect inclusions), as e.g.
most encoders don't need it. It is furthemore in preparation
for moving the private part of AVCodec out of the public codec.h.

a247ac640d avcodec: Constify AVCodecs 
Given that the AVCodec.next pointer has now been removed, most of the
AVCodecs are not modified at all any more and can therefore be made
const (as this patch does); the only exceptions are the very few codecs
for external libraries that have a init_static_data callback.

By switching to ff_init_vlc_from_lengths() one can replace tables of
codes of type uint16_t with tables of symbols of type uint8_t, saving
space.

16bb8247b4 avcodec/mpegaudiodsp: Make initializing synth windows thread-safe 
These arrays are used by the Musepack decoders, the MPEG audio decoders
as well as qdm2 and up until now, these arrays might be initialized more
than once, leading to potential data races as well as unnecessary
initializations. Therefore this commit ensures that each array will only
be initialized once.

158f7df5e6 avcodec/mpc7, mpc7data: Avoid gaps in array 
The Musepack decoder uses static VLC tables to parse the bitstream.
There are 14 different quant tables VLCs and each of them has a varying
number of codes. The maximum number is 63, the average number is 25.3.
Up until now, the array containing the raw data was of type
uint16_t [7][2][64 * 2] (the 14 tables come in pairs of two, hence [7][2]
instead of [14]) and from this it follows that there were large gaps in
said array. This commit changes this by making it a continuous array
instead. Doing so saves about 2KB.

d7eabd5042 mpc: pad mpc_CC/SCF[] tables to allow for negative indices. 
MPC8 allows indices of mpc_CC up to -1, and mpc_SCF up to -6, thus pad
the tables by that much on the left end.
